Method - Telephone interviews were conducted between January 12, 1998 and April 20, 1998. Random selection of 800 cases from universe of 47,284 cases. Notifications were sent out to all respondents, but 161 cases were returned undeliverable and 89 were disqualified because addresses were P.O. boxes. 292 interviews were completed, a response rate of 36.5% from the original list of 800 cases.
