Center for Health Care Strategies (CHCS) is a policy resource for information for improving the quality and effectiveness of health care for low income people and those with chronic illnesses and disabilities. http://www.chcs.org/ Partnership for Prevention is an initiative that focuses on improving the care and quality of life for those with chronic condition. Robert Wood Johnson Foundation supports efforts to improve the quality of care that Americans with chronic illness receive.
