| Arizona | Arizona HB 2707 and SB 1470 Requires applicants applying for public healthcare to disclose the name of their employer. Died |
| California | California AB 89 Requires the Department of Health Services and the Managed Risk Medical Insurance Board to convey to the Legislature a report identifying all employers who employ 25 or more persons who are beneficiaries or who support beneficiaries of the Medi-Cal, healthy Families and the Access for Infants and Morthers programs. These reports must be made available to the public. |

Georgia HB 548 Provides for the compliance of data relating to an individual’s employer for applicants and their dependents receiving health care benefits under the state Medicaid program, PeachCare for Kids Program, or any other state funded or administered health care program. |
| Hawaii |
Hawaii HB 334 Requires the Department of Human Services to submit an annual report to the Legislature on private sector employers with more than 50 employees who receive medical care or benefits from public assistance programs. Hawaii SB 1772 Requires each applicant for medical assistance to identify the employer of the proposed beneficiary of medical assistance; requires the Department of Human Services to submit an annual report to the Legislature identifying all employers who employ 25 or more beneficiaries of medical assistance programs. Vetoed by Governor, veto overrode 7-12-05 Hawaii HB 336 Requires Department of Human Services to annually identify employers of applicants for medical assistance programs and disclose them to the legislature. Requires report to include cost of medical assistance to these applicants. Requires DHS to make public these employers without identifying individual applicants. |
| Iowa | Iowa SB 199 Requires an individual who is the recipient of health care services in a hospital and is uninsured to disclose to the hospital the identity of the employer of the proposed beneficiary of health care benefits. Also, an applicant for the medical assistance program shall disclose the identity of the employer of the proposed beneficiary of health care benefits. Also provides for reporting to state agencies and the legislature. |
| Illinois | Illinois HB 1044 Provides that an applicant for benefits under Medicaid or the Children's Health Insurance Program, or a person requesting uncompensated care in a hospital, must identify the employer or employers of the proposed beneficiary of the health care benefits provided under that program. Requires the Department of Public Aid to submit to the General Assembly an annual report, beginning in 2007, identifying all such identified employers who employ 25 or more public health program beneficiaries. | Maryland | Maryland HB 791 Requires an applicant for specified State health programs to identify the employer of a proposed beneficiary or the employer of a specified family member of a proposed beneficiary. Also requires the Department of Health and Mental Hygiene to publish a report identifying specified employers and providing specified information. The report must protect the confidentiality of individuals in the State health programs. Finally, the report must be made available to the public. |

Ohio SB 103 Requires applicants for CHIP, Disability Medical Assistance and Medicaid to provide information about their employers, to require hospitals to gather employer information from patients who receive treatment under the Hospital Care Assurance Program, and to require an annual report identifying the employers. |

Oregon HB 2606 Requires Department of Human Services to publish an annual report containing lists of employers with 25 or more employees with publicly funded or publicly subsidized health care. Oregon HB 3055 Requires Department of Human Services to publish and submit to the Legislative Assembly an annual report containing lists of employers with employees receiving public assistance, including food stamp benefits. | Virginia | Virginia SB 742 Requires the Department of Medical Assistance Services to collect from the Department of Social Services any available information regarding full-or part-time employment of any applicant for health care benefits under public health programs, such as Medicaid, the Family Access to Medical Insurance Security Plan, and the State/Local Hospitalization Program. The Department of Medical Assistance Services will prepare and submit a summary to the Governor and the General assembly of statistical data on the full- and part-time employment of applicants for public health programs and the costs of the beneficiaries' health care to Virginia, which will classify Virginia employers by categories, as determined by the Director.
