This table is intended to provide general information and does not necessarily address all aspects of this topic. Because the facts of each situation may vary, this information may need to be supplemented by consulting legal advisors. It reflects in summary form statutes in effect as of 12/31/11 or statutes set to take effect shortly thereafter. This research list states' disclosure requirements. It does not comprehensively list bans states may have on practices other states require to be disclosed. For example, one state may ban honoraria, while another requires legislators to report any they receive. Generally, states do not require filers to list their primary homes as property. In many cases, they don't require them to list other recreational homes either. Generally, states exempt privileged information, such as clients in industries where confidentiality is a priority, from income reporting requirements. In many cases, they exempt individual sources that constitute a part of gross income. When this exemption is present, it is not always noted in the table.

For all sources of income over $1,000 including taxable capital gains received by the person, the person's spouse or domestic partner, or the person's dependent child, must provide:
source of the income; the recipient of the income; the amount of the income
a brief statement describing whether the income was earned by commission, by the job, by the hour, or by some other method; the approximate number of hours worked to earn the income; and unless required by law to be kept confidential, a description sufficient to make clear to a person of ordinary understanding the nature of each service performed and the date the service was performed.
Income in excess of $1,000 received as compensation for personal services, the name and address of the source of the income, and a statement describing the nature of the services performed; if the source of income is known or reasonably should be known to have a substantial interest in legislative, administrative, or political action and the recipient of the income is a legislator or a legislative director, the amount of income received.

The name and address of each business in which the person, the person's spouse or domestic partner, or the person's dependent child has an interest or was a stockholder, owner, officer, director, partner, proprietor, or employee during the preceding calendar year, except that an interest of less than $1,000 in the stock of a publicly traded corporation need not be included.
A public official may transfer all or a portion of the official's assets to a blind trust for the duration of service in public office. The original assets placed in the blind trust shall be listed by the official in a statement filed under this section, together with a description of the actual or potential conflicts of interest, or appearance of conflict, that the official seeks to avoid by the use of the trust. A copy of the instrument creating the blind trust must be included with the statement. 30.50.40 provides for a variety of other conditions that must be met.

Option two: All sources of income to a business entity in excess of 10 percent of the gross income of a business entity in which the reporting person held a material interest and from which he or she received gross income exceeding $5,000 during the disclosure period. The period for computing the gross income of the business entity is the fiscal year of the business entity which ended on, or immediately prior to, the end of the disclosure period of the person reporting.

Income is defined as economic gain to a person from any source, including, but not limited to, compensation for services, including fees, commissions and payments in kind; gross income derived from business; gross income derived from dealings in property, rents and royalties; gross income from investments including interest, capital gains and dividends; annuities; income from life insurance and endowment contracts; pensions; income from discharge of indebtedness; distributions from a partnership or LLC, gross income from an interest in an estate or trust; prizes; and grants.

If associated with a corporation, partnership, LLC or other business in which the reporting individual or immediate family members own or control, directly or indirectly, more than 50% of the outstanding equity, that has received revenue of $2000 or more or professional association, include its major areas of economic activity.
Any offices, trusteeships, directorships or positions of any nature, whether compensated or uncompensated, held by the reporting individual or immediate family member with any for- or non-profit, corporation, association, LLC, partnership or business.
Each source of income of $2000 or more derived from providing attorney services, the major areas of law practiced, and if associated with a firm, that firm's major areas of practiced.
